Integrated Care Pathway (ICP)
We have partnered with the team at HABIT HEALTH to deliver physiotherapy services in Wanaka through the Integrated Care Pathway (ICP). This is a fully funded rehabilitation pathway to help individuals with a significant injury have access to a higher standard of care and help them achieve their goals more effectively.
ICP provides faster access to Sports Medicine Specialists, Orthopaedic Surgeons, Occupational Therapists and other members of the MDT to achieve a cohesive approach to rehabilitation. Reducing wait times and wasted money of a traditional fragmented approach.

The aim of ICP is to:
- reduced risk of re-injury;
- reduction in unnecessary treatments;
- improved equity and access for Maori and pacific; and
- more timely access to surgery and rehabilitation.
Lower back (Lumbar Spine)
- Lower back pain with neurological deficit
- Lumbar spine compression or other fracture
Knee
- Haemarthrosis – ACL or patella dislocation
- Meniscal signs – Catching, locking or clicking
- Confirmed fracture – tibial plateau, patella, distal femur
Shoulder
- Significant mechanism of injury/trauma (dislocation, fracture or AC Joint grade 3)
- Full rotator cuff tear (confirmed via USS)
GP involvement
You can refer clients with appropriate knee, shoulder and lumbar spine musculoskeletal conditions. They must meet the entry criteria outlined below and must live in the Wanaka area.

* All injuries put forward for funding through ICP must have an accepted claim through ACC with approved claim number.
* Injuries must be within the past 12 months.
* You must not have had “elective” surgery. Make sure you contact us prior to any surgery for this claim or you risk not getting funding.
